Exam Scheduling Form

Exam forms must be submitted at least two weeks prior to the exam date. This allows time for the Graduate School to conduct a degree audit (for final exams), and for you to resolve any unexpected issues prior to your exam.

If you are submitting this form late its approval is not guaranteed. All late submissions are reviewed on a case-by-case basis.

When you submit the form, a PDF copy is automatically emailed to the Graduate School and to your OSU email address. We will notify you as soon as possible if we can approve your exam for your chosen date.

All master’s and doctoral students use this form except dual-majors who use the PDF version found below.

For students with dual-majors, please use the PDF form below and follow instructions at the top of the form. When complete, email your PDF form to [email protected]

Instructions:

  1. Save the Exam Scheduling Form to your computer.
  2. Open the form in Adobe Acrobat Reader (not in a browser like Chrome or Firefox). Adobe Reader download instructions below.
  3. Fill out all required questions (marked with *).
  4. Click the “Submit” button on the form.

Mac users: Set your default email program to Mac Mail (Gmail and Outlook will not work).

Adobe Reader download instructions: Download and install the newest (free) version of Adobe Acrobat Reader. This form does not work with Preview or in a browser.
