Program of Study Form

As a regular graduate student, you must file a Program of Study with the Graduate School. Students who do not file a Program of Study within the specified deadline for their degree will not be allowed to register for the next term.

Students pursuing a dual-major master's or a dual-major doctoral degree must use the following PDF form provided by the Graduate School. All other students must login to the digital program of study to complete, sign and submit your Program of Study. When logging in, please use the OSU Login Button. To make changes to an already approved digital Program of Study, please use the Petition for Change in Program of Study form.
