Nomination Form for Flyfisher's Scholarship
Departments and programs may submit only one nomination and must provide complete document packets containing all materials listed below (including transcripts.) Incomplete nominations will not be reviewed.
Please use this form to upload the following materials as one (1) PDF document:
- a letter of nomination from the department head or chair or interdisciplinary program director that demonstrates the candidate's merit and the relevance of his or her research focus as meeting scholarship eligibility criteria;
- two additional letters of recommendation;
- the student's curriculum vita or résumé;
- the student's statement of academic objectives; and
- copies of transcripts of all prior work must accompany the nomination. [Note: transcripts must be provided by the nominating department or graduate program.]
